What to include in the referral

Agencies referring to SHED should provide as much information as possible, as this will ensure a more effective and speedy response. Include:

• Most up to date common assessment if available
• Completed substance misuse screening tool
• Any other support agencies involved with the young person and
  their contact details.
• Contact details for the young person - it is essential that a telephone contact be given if possible as this is the most likely first route of contact. Also make clear any issues about contacting the young person.
